Accountable to: Transportation Manager / Director of Transportation
Qualifications:
General - Certified by a Nevada licensed physician within 6 months prior to employment to be in good health including freedom from communicable disease and no physical limitations. A 2-step TB skin test is required to be completed prior to start of employment, followed by an annual 1-step TB skin test or a blood test called Quantiferon Gold ordered by a physician. If presented with a documented history of positive skin test, a one time CXR and an initial TB Questionnaire must be completed. The CXR must show no active TB and the Questionnaire with no symptoms of TB. The TB Questionnaire will then be required annually thereafter. A 10-panel Drug test is required within 30 minutes after job offer with a negative result. A 7 year history of criminal background check via Occuscreen will be performed after a negative Drug test is received. Applicant/Employee will be required to submit a self-requested printout of their 10 year MVR history from the DMV and may have no more than 1 demerit point. Reports showing convictions may require approval for hire from contracted brokers (RTC and MTM).
Education/Experience - Must have a current Nevada Driver's license (CDL is required for operation of a 15 or more passenger vehicle or equivalent). Minimum of 1 year experience in transportation services is required. Experience in transportation of the elderly and/or physically and mentally challenged adults preferred. Must be able to communicate in English. CPR & First Aid certification required within 30 days of hire. Must be able to lift at least 50 lbs. without any restrictions. Must be able to bend and squat, push and pull wheelchairs. Must be at least 25 years of age per insurance requirement.
Abilities - In addition to the responsibilities detailed in this job description, the Driver must be able to:
1. Understand the needs of the elderly and, in particular, the needs of those with cognitive impairments (memory problems, confusion, disorientation, intellectual and developmental disabilities).
2. Be sensitive to the needs of family caregivers.
3. Be available to work unscheduled days and hours for coverage
4. Be able to take on extra trips in addition to regularly scheduled trips as needed.
5. Demonstrate good communication skills.
6. Demonstrate knowledge of the routes used in transporting clients and of roadways in the Las Vegas/Henderson service areas.
7. Demonstrate the ability to read and to understand a road map or mapquest or GPS.
8. Make a commitment to cooperative performance to attain care, organization programmatic objectives which may entail performing duties not included in this job description.
Responsibilities of the Driver
The Driver is responsible for assisting and safely transporting clients to and from the Center. Responsibilities include, but not limited to:
1. Performing Daily Pre-Trip Checklist of vehicle.
2. Reporting immediately to the Director of Transportation, Transportation Manager any problems related to the vehicle, route or scheduling;
3. Reporting immediately to the Program Director/Administrator/CEO/COO/Nursing any problems observed or encountered while transporting clients.
4. Safe assistance and transportation of clients include escorting door to door, following the laws on the road, and making sure clients are received by a family member or authorized designee when being dropped off, unless authorized to be dropped off alone (Authorization Form must be signed and on file).
5. Any citations due to violation of the laws on the road will be the responsibility of the driver, including payment of bail amount.
6. Responsible for cost of repairs for damages to property &/or vehicle caused by careless driving or failure to follow warning signs to avoid damages.
7. Responsible for handling clients' personal belongings, medications and family communications which must be conveyed from the clients' residence to the Center and vice versa.
8. Responsible for collecting transportation fares from clients (if applicable) and submitting fares to Transportation Manager upon receipt.
9. Maintaining good rapport with family caregivers and participants.
10. Maintaining good rapport with the community.
11. Daily cleaning of the interior of the vehicle.
12. Emergency evacuation of participants if inclement weather is threatened or if there is a power failure or other reason that the Center must close.
13. Handling of emergencies while en route until emergency assistance arrives at the location of the driver including calling 911, as needed.
14. Responsible for vehicle keys, cell phones and other accessories issued to perform the duties. Cost to replace any loss of any equipment will be the responsibility of the driver.
15. If illness occurs during transport, driver must immediately park at a safe location and contact the Transportation Manager/Lead Drivers, remain with the client(s) in the vehicle until relief arrives to take over the route.
NEVER LEAVE CLIENT(S) UNATTENDED WHILE INSIDE THE VEHICLE OR LOSE VISUALIZATION OF THE VEHICLE DURING PICK UP OR DROP OFF.

Qualifications: General – Certified by a Nevada Licensed physician within 6 months prior to employment to be in good health including freedom from communicable disease, including tuberculosis in the communicable form, according to acceptable screening methods of the Department of Health Division.
Education/Experience – High School Diploma. Prior personal or work experience with the Developmentally Disabled / Autistic Spectrums, Behavioral modification experience preferred.
Abilities – In addition to the responsibilities detailed in this job description, the BHT must be able to:
1. Understand the needs of the young Adults with Developmental Disabilities and, in particular, the needs of those with cognitive impairments, mental illnesses and Traumatic Brain injuries.
2. Be sensitive to the needs of families and caregivers;

4. Make commitment to cooperative performance to attain care , organization and programmatic objectives which may entail performing duties not included in this job description.
Responsibilities of the Behavioral Health Technician/DSS
The BHT/DSS is responsible for providing personal care under the supervision of the Lead Behavioral Health Tech & Program Director and for assisting with Activities of Daily Living, meals, activities, behavioral modification, behavioral interventions, activities and housekeeping functions of the program. Responsibilities include:
1. Observation of each participant listen to concerns, record their observation and report immediately any changes in their condition or any unusual behaviors to Lead BHT/DSS, Program Director and or QIDP/Nursing.
2. Under the supervision of the Lead BHT/DSS, QIDP/Nursing, or Program Director, assisting with Activities of Daily Living including taking participants to the toilet that need
assistance at scheduled times (every two hours) which includes continent care, transferring or lifting from wheelchair to commode or changing bed.
3. When behavior changes, BHT/DSS must be able to assess whether the changes are negative, positive or dangerous and take steps to modify participants actions.
4. BHT/DSS has responsibility to maintain safety for themselves and the participant. This may involve restraining violent participants with caution and assistance, and should ALWAYS be the last resource.
5. BHT/DSS must be able to emotionally support participants, remain calm, professional and use interpersonal skills to defuse a volatile situation.
6. Assisting with ambulation those with assistive devices such as walkers, cane and wheelchairs.
7. Housekeeping duties to prevent the spread of infection and sanitation of facility.
8. Taking care to prevent breakdown of the participant’s skin.
9. Assisting with meal service and assisting /feeding assigned participants to eat their meals and informing Nursing when participants are not eating or adequately or taking fluids.
10. Documentation on Intake and Output log for each client.
11. Assist in emergencies as directed by the Lead BHT/DSS, Program Director and QIDP/Nursing.
12. Daily straightening of the participants personal belongings in storage cabinets.
13. Assisting with program activities when not performing the above duties.

A 7 year history of criminal background check and Motor Vehicle Report via Occuscreen will be performed after a negative Drug test is received, with an acceptable result per Medicaid regulations. MVR may have no more than 1 demerit point and may require approval for hire from contracted brokers (RTC and MTM). All must be done upon hire and annually thereafter.
